Need help adding a "comment/note" feature

I have a database that is based off of the support ticket template from MS
and what i need to do is implement a feature that allows you to add notes or
comments to each ticket. But this shouldnt be confused with description; i
want the user to be able to add notes when they want to update the ticket
with some detail/status.

So that means, there can be multiple notes/comments. Does any one have a
ticket database or something similar with this functionality that i can learn
from? I dont even know where to start. Thanks!
