Need help making a query

Greetings

I work for a transportation brokering company. We move
liquid chemicals around the country.

I have created a large table with our 'load history'
information. It consists of 12 columns labeled like:

PMI # - Customer - Carrier - Origin - Destination

There is one record for each load.
I need to create a data access page where the user can
type in the origin and destination (or selct from combo
boxes)and the query responds with all the loads listed
for those points and displays them in a pivot table.

I know this is probably very basic but I'm very new to
access and the language.
