Need to allow folks to attach a file to my online web form

One of my webpages is a fill-in-the-blank form and I need to add a feature so
that people can attach a word file before submitting the form to me.

Can someone help me on how to do this?

You must be on a Windows server with a host that supports the permissions required for File Upload
