Need to get info into a calendar from an appt form with custom fields

I have created a custom form with custom fields bound to controls that the
user updates (I made the second page, P.2, my main page that pops up when
it's opened). But, when I save the appointment, it shows up blank on the
calendar, and not at the time that the user has put in. How do I get the
data in my controls / custom fields to appear how I want it on the calendar?
Better yet, how do I even get it to save at the time that the user inputs,
and show up with information of any kind in it instead of blank? (Yes, the
calendar is open when I save the form.)

Okay, so I figured out that I need to use the built-in fields for
Apponitments, but now I have two questions:

1.) How do I get the appointment to save to a specific calendar, and not my
personal calendar? I have tried having only the specific calendar open when
I "Save and Close" the appointment... doesn't work... I have tried going to
Tools->Forms->Choose a form when the specific calendar is open, and also
going to New->choose a form when the specific calendar is open, neither work..
. still gets saved to my personal calendar.

2.) How do I format the appointment to look like I want it to on the
calendar? It automatically puts Subject, Location, Organizer in the entry on
the calendar, but what if I want the appointment to show up with the
Organizer, a different user-defined field, and then the subject?

Another issue I've run into is the pesky reminder that goes off as I hit the
"Save and Close" button, even if the appointment isn't for hours... but I'm
hoping that will right itself when I am able to save it to the right calendar?
?
