New appointment info screen does not appear?

When I click New Appointmnet the info screen does not appear? What setting
have a inadvertantly clicked?

Brian,
When you click new appointment, an information screen is suppose to open
that allows you to describe the subject, location, date time and general
info. Also enables you to invite others
