New Contacts Have Notes Icon. How to Fix?

Outlook 2003. I have created a new contact form (by editing the default
Contact form provided by Outlook). When I create a new contact using this
form, the small icon displayed next to the contact in the righthand pane of
the Contacts window is the icon for a note. How may I fix this to display
the same icon as my other contacts (which looks like an index card with a
picture on it)? Also, I changed the message class of all my eixsting
contacts to point to my new form. Their icons are okay. Thanks for any help.

P.S. I have seen the "Change Large Icon" and "Change Small Icon" on the
"Properties" tab in design mode. These both show a Notes icon. If this is
where I set it to the Contacts icons, then where on earth do I find these
icons? Thank you.

Correction. The icon displayed for the new Contact is a "Post It" type icon.
I found the large and small Contacts icons in C:\Program Files\Microsoft
Office\OFFICE11\FORMS\1033\Contactl.ico and Contacts.ico. I changed to these
in the properties tab in design mode, but new Contacts still have the "Post
It" icon. Help.

Okay. Once I changed the Large and Small icons in the Properties tab of the
Contacts form in design mode and published the form, the change did not take
effect until I had closed out of Outlook and come back in. Now my new
contacts have the small Contacts icon showing when created.
