Not sure which wizard you are using.
When I use a wizard to create a form, it allows me to select which table to
use, then which fields in that table. When I select from the fields
available to show "fields used", they appear in the finished form.
If the wizard just doesn't want to work, you could build your form the
old-fashioned way. Open a new form in design view, set the Source for the
form to your table (better still, to a query against the table), and start
dragging the table's fields onto the form.
