OLE Server error

I have a subform used to attach documents. The subform works fine, able to
upload the attachment and view it. When I close the attachment, I receive an
OLE Server error recommending I reinstall the server because it couldn't be
found. I'm able to view it, just need to get rid of this error. It's
happening on two separate PC's.
