OneNote help indicates that OneNote button will be in email, contact, and
other Outlook components, but I haven't found it, nor have I found a way to
add it to the toolbars by the customizing toolbars commands. I am using XP
and Outlook 2007.
Any suggestions?

Which verison of Office do you have installed? Does it have OneNote
installed on it?

Yes, I ran the Diagnostics. No change there.
Running MS Office 2007 with OneNote installed.

Normally running Diagnostics will not fix problems.

Try it with running a Repair for both Outlook and OneNote:
Control Panel | Add or Remove Software (XP)
Control Panel | Programs and Functions (Vista).

P.S. Are you really sure to look at the right places?
An ON-icon does not appear in the toolbars of Outlook.
There is a Ribbon item, however, when mail, calendar and contact
elements are opened in the edit mode.
