Online Survey Tools

I know this probably isn't the best place to ask but since there isn't a
group dedicated to survey/questionnaires I thought I'd start here. I'm
looking for an online tool for administering Questionnaires but I also need
to be able to score the questionnaire and give immediate feedback to the
participant. This gives several advantages, One the data does not need to be
completed on paper and then entered into a database, Two the data is
centralized for statistical work and research and Three the participant gets
a score in real time instead of trying to learn to score the questionnaire.
Currently the questioners is only in paper copy and trying to centralize
things would take a lot of work.

I've looked at Survey Monkey but it will not allow me to score the survey
for the participant. Any ideas if an online tool exists?

There is a difference between surveys and exams. Surveys are to gather
information - not for scoring. Perhaps you are searching under the wrong
term to find providers. Search under exam or test I would think to get that
kind of functionality.
