Online surveys with Access

Hello All:

OK, I'm web challenged. I need to create an online
survey for my organization. They want it to be "password
protected" so that users have to login with an ID and
password to complete the survey, AND they want it to be
linked to our website (we use FrontPage for that). My
questions are:

1) How do I create a database that requires a login to
use (they'd like another higher level login for
maintenance of certain areas, but we can push that down
the road a bit), and;

2) What do I need to do to use the Access database as a
functional link off a FrontPage 2003 web site.
