Open Input Box

Hi,

I was wondering if anyone can tell me how to enter a macro or feild
that allows a word template/document to open a dialogue box apon
opening the document. You would need to enter eg you name and it
automatically puts into the designated location for you.

I have seen this happen but need to know how to create one myslef.
