Opening a form with default values

I'm opening a form from a switchboard in edit mode and cannot figur
out how to open with empty text boxes. Can anyone help?? I do hav
the default value set as blank

This might be explained better as opening a form to a "refreshed" o
"blank" state

If I need to explain myself better or you need more info, please le
me know. Thank you

Thank you for the tip...

Opened the code to the switchboard and plugged this in where the for
opens. Everything is working great now
