Operation Failed - Sending Outlook Form

Hello -

I have multiple client systems that when they try and send an Outlook
form they are unable to send and get a generic Operation Failed error.
I have tried to clear the Forms cache (no luck) and ensured
Permissions and Security are correct - does anybody have any articles
or ideas on where to start troubleshooting?
