Others can't read my 2nd page

I have a Word document that I received which was a form designed to be
printed on legal sized paper with color.

I used Microsoft Word for Mac 2011 to open, and I filled it out then I
sent it to someone who needed to review it. She wrote back that she
only got the first page. She said maybe that's because she has a Mac
(obviously that's not the problem as I have a Mac as well - but I
suspect she doesn't have Word for Mac 2011).

So I opened it with Word and saw 2 pages.

I opened it in Preview, Pages, & Neooffice, and only saw 1 page - the
last page was blank.

So I edited off the first page, saving only the last page. I looked
at in Preview and saw the text without the formatting. I sent it to
her, but I'm wanting to debug my problem. How do I do this
debugging?

This not be a complete answer, but it gives some insight into possible
problems with sharing Word documents (because of the proprietary format):
