Out of office?

Does the Out of Office auto-responder in Outlook work even if the mail
server is a simple SMTP (e.g. Windows Server 2003)?

Outlook 2003 if that makes a difference.

Out of office only works with a Microsoft Exchange server. You can simulate
the behavior by using an Inbox Rule, but Outlook has to remain running in
order for it to work with POP3/IMAP accounts.
