Outlook Bar Resets to Default

My Outlook bar used to work fine. A few months ago, I
opened Outlook (2000) and all the icons in the Outlook bar
were reset to the default ones, and as large icons. I
would take out the ones I didn't use, and add in my
important ones. Each day, it would reset to the default
icons again at startup.
Any idea how I can get my added folders to stay in the
view I would prefer to have them in my Outlook bar? This
would greatly help my productivity!
