OUTLOOK - CHECKBOX

I create a form/template and place a checkbox control on
the form.

One instance, I attach the control to a field and get a
gray shaded checkmark in the box and when I try to run
the form, I cannot change the checkmark to dark or even
select the box. When I send the form, only the shaded
checkmark shows.

In the second trial, I do not attach the checkbox to a
field, when I run the form I can 'mark' the box and see a
checkmark, but when I receive the form, the checkbox is
empty. What is missing in the design of the form.

A check box needs to be bound to a Yes/No (i.e. Boolean) data field. What
field did you bind your check box to?
