Outlook Connector appears to uninstall itself

I have installed Outlook Connector. The first time I use Outlook after
installing Outlook Connector it appears installed (Outlook Connector is
included in the MenuBar and the bottom of the window includes different
info), but every time after that Outlook Connector appears uninstalled
(Outlook Connector does not appear in the MenuBar and the bottom of the
window does not include the same info). Why does Outlook Connector keep
disappearing?
