Outlook folder loses its association with the InfoPath form

I created an InfoPath folder in Outlook to view form data. Promoting form
properties worked flawlessly and allowed me to view my chosen fields.
However, when I close and re-start Outlook, the InfoPath folder loses its
association with the form. I have to promote form properties again each time
I open Outlook. As a test, I opened the form in InfoPath design mode and
removed all of my selections from the property promotion tab. Now the Outlook
folder will retain its association with my form. Of course, I don’t have any
of the fields I need to review in Outlook. When I go back to property
promotion in InfoPath design mode and add my fields, the Outlook folder will
once again lose its association with the form. I have tried creating a new
pst folder in Outlook with no luck. My exchange account is set at cached
Exchange mode. The same problem occurs on networked PCs at work as well as my
home pc. Does anyone have a suggestion on how I can resolve this?
