Outlook Forms - Contacts

Hello,

I have been trying to create a form within outlook
contacts. The field is similar in function to the
anniversary or birthday fields. I want to have a date
field which is posted to the calendar like the birthday
or anniversary fields on the details tab in contacts. I
have tried to use these fields from the filed chooser but
even if I change the label on the form I cannot change
the label when it posts to the calendar. It still
says "contacts name" and birthday or anniversary. Any
suggestions?
