I've been asked to create an e-mail questionnaire and I'm
wondering what is the best to create it? I want to make
sure that most e-mail clients will be able to read it and
input answers. Most questions will have a numeric answer--
1 to 5--but additional text input will be needed as well.
Should a form or template, or something else be used?
