Please need help in Outlook Forms. I have designed a form for work

The form is designed. However, I am facing problems when inserting

Information on the form for e.g. name, number etc and send TO...

The receiver does not receive insert information on the form.

Make sure you're following all the steps necessary to ensure that the recipient gets your message in rich-text format and has access to the form definition. The details depend on your version of Outlook and mail configuration. Also the form may need a read layout with controls to display your custom fields. I have a similar problem. I have created a new form for a
public folder. Once the form has been filled in and
posted to the folder some of the e-mail headers inside the
form disappear. So you go into the e-mail and it does not
display some of the information. I have found that if you
go into Edit and then revise contents you can view it.

Dont know whether this helps.

Sounds like you might have designed only a compose layout and not a read layout.
