Outlook keeps asking for my User Name & Password to check email?

Outlook 2010. Outlook keeps asking for my User Name & Password to check email as if the information was never entered before. I have been using Outlook for my email for several years and nothing has changed. For some reason every 6 months or so Outlook has usually come up and asked me to enter myuser name and password as if they have never been entered. Usually I can just click the Okay button without entering anything as the suggested information is correct and Outlook will resume normal operation. Lately that dialog box keeps coming up. How do I get rid of it?

The usual cause of this is not Outlook, it's your ISP's mail servers asking
the login details. Take a look at any newsgroups/forums specific to your
ISP
