Outlook Modifications

Hi,

I have been asked by a client to set up a Public Folder for their contacts
and would like modifications made to the standard Contact Form.

All of the modifications have been done and are working well.

The issue I have is that I have is when I click on New, the contact page
displayed containing the details of a Test Contact User that I set up. I have
deleted all the test details in the folder but this problem still exists.

Essentially I would like the contacts form to be blank when clicking on New.

Any suggestions?

2. How did you create the forms?
3. Where are they published?
