Page numbering errors in TOC

We created a word document with many chapters - with section breaks between
chapters. The page numbering for each chapter is unique per chapter - i.e.
Chapter 1's page numbers are 1-1 while Chapter 2's are 2-1, etc. Most of the
time, this works correctly when creating the TOC. However, there are a
couple of errors, for instance the first page of Chapter 3 is listed on page
1-10 in the TOC. When I go to the first page of Chapter 3 in the document,
it is correctly on 3-1. Also, on one page there are 3 section headings, the
first 2 are listed correctly on page xiii in the TOC but the 3rd and last
section heading is listed on page 1-13 in the TOC. The section breaks appear
correct. Any thoughts would be helpful as there are a number of us in the
office that are confused.

My guess would be that you have used a manual page break somewhere and it is
messing up the pagination.

nope - no manual page breaks, but why should that create the pagination
problem? The page numbers at the bottom of the pages are correct, but the
ones being pulled into the TOC are not always correct - mostly, but not
always.
