Partially Protected Document

I want to create a document in which some contents are to
be protected but some are not. The desired document would
be like this:


BMCC Document

Department:__________________

Curriculum:__________________


BMCC Document, Department, and Curriculum need to be
protected or users can not edit or make changes. But two
underline fields are not protected, that means users can
fill in.

What can I do or what is name for this kind of document?
