Password Prompt Form

I need to create a password form so that when the user clicks on a command
button a form appears prompting the user to enter a password.

When the password is incorrect a message box should appear exampling what
happened.

And if possible I would like to create a table that logs the time, date, and
password used each time an incorrect password is entered.
