PDF Form Field Values

Is there a way using vba to simply extract the user input values from a PDF
form so they can be 'Uploaded' into a database?

I don't know about the PDF per se, but you can certainly do this in Word.

If you have MANY PDF files that you need to do this for, look into
converting them Word docs:
http://www.pdftoword.com/

Then follow the steps in the first link I posted.
