Picture in Submit form

Having problems making this work..... Everything is good
with the server. Trying to put a file upload to submit with
a form - save to Database so you can view picture with
info.....
Well........ tried it..... I have another from that people
can submit a picture with that emails it to me..... It
works fine.. But when I try to get it to send with the new
pages I get an error "the document contains no data"
Not sure why.... submit page is
