Populate dropdown menus from DB

Hello, I have a database that I am trying to run a query
on through dropdown boxes. The test pages is located
here....

I want the 3 drop downs populated dynamically, and the
submitted for the database query.

If you take a look at what I have, every time you submit
the dropdown it does pull the right table to the next menu
but resets the above.
