Preserving vertical alignment with fields and text

I am trying to create a form that can be filled out on the
computer and then printed. I am putting fields in to
prompt users to input data but as data is input into
fields that wrap around to another line it pushes the
information below it out of alignment. I've tried to put
the field in a table but now it runs off the right edge of
the paper then wraps around....

Help!!!

Turn off the option in the Table properties that allows the
columns to "fit to content"? And set the row height to an
exact amount, as well.
