Print Contacts in DaytimerJunior Desk format both sides of page`

Have an old HP Laserjet 4P printer and want to print my Outlook 2003 Contacts
with
the addresses/phones/email addresses info in Day Timer Junior Desk format
and on both sides of each page. Did it four years ago but have forgotten.
