Print formatting tri-fold brochure - help!

Could anybody possibly help me? I need to somehow print format the attached document so it will print out on A4 paper, as a tri-fold brochure. I keep failing, and can't seem to find out why or where!

If anybody could please assist, it would be greatly appreciated.
