Why do you need a template? Start with a blank card - type your word -
print.

You can use mailing labels for this. In Word, use the Envelopes and Labels
dialog box, select Labels, and in the Avery labels, find the Product number
3588. Once found, select it and then click New Document. The end result is a
Word table with the structure for 3, 3 x 5 index cards. To add more labels,
press Tab in the last cell of the table.

Now, if you are trying to print on index cards all you need to do is create
a custom paper size using 3 X 5, you'll probably also need to change your
page orientation to Landscape. Whether your printer will actually print them
depends on the printer you are using.
