Printing 2 column vakues from a Lookup Table

Hello:

I am using a 2-column Lookup Table that has a code and the
location as the 2 columns.

I created a report where the user running the report
enters the code, so the report will only show entries that
meet the criteria entered.

It works no problem, but is there anyway to have BOTH
columns appear? If I have to switch to 1 column to make it
work that's OK, as I am still in the testing phase.
