Printing Address Labels from Outlook 2000

I am using Office 2000 in WXP Pro. When I print an
address in Word from the Outlook address book, it prints
NAME, ADDRESS, CITY, STATE, ZIP, but omits the company
name. How can I get it to print the address as:
NAME
COMPANY
ADDRESS
CITY, STATE ZIP
