Problem keeping templates consistent

We received technical reports from different sources (government, industry,
and academia) that follow a supplied custom template. But sometimes they do
not follow or use the supplied template. We have a huge problem trying to
either fix, update, or attach the correct template. We end up reformatting
by resetting the styles again and again because after saving the file and
reopened at a later time, the styles have either reverted back to the
original style settings or it picks up styles from somewhere else. We've
tried cleaning out the styles, using the organizer to copy the correct styles
into the problem document, de-select automatically update, and attaching the
correct template to the file but to no avail.

What is the correct procedure to follow that will eliminate this problem?

Create a document from the appropriate template. Then use Insert | File to
bring in the incorrectly formatted document. You can select the whole
document (press Ctrl+A) and then press Ctrl+Q and Ctrl+SpaceBar to clear any
direct formatting applied.

Note, however, that if your clients applied different styles to text, you
would have to manually re-apply the correct ones.
