Protected document becoming unprotected

I am activating document protection with a password to allow track changes
only. The document is coming back unprotected and I am unable to see the
edits made. The reviewer has no way of knowing the password. What is
happening here?

is it really unprotected? maybe unchecked Tools > Track Changes > Highlight
Changes... > Highlight changes on screen ? (W2k)
