[PS2007] - Problem with Issues and Risks in PWA

Hi,

I have this problem recently; when I click on the Issues and Risks Item in
PWA, I received error "Your request has failed for an undetectable reason,
you may need to contact your system or database administrator."

Does anyone have any thoughts on how to resolve this?
