Publishing a Custom form: OL2002 sp2

Greetings!

I've seen many postings similar to my issue but nothing that seemed to
pinpoint my problem.

The custom form (OL2002 SP2) in question utilizes VB script to print
the form via a MS Word template on both the compose/edit pages and
works flawlessly from my Personal Forms Library or an OFT. However
once our Exchange admin published the form to the Organization Forms
Library, my VB script print button ceased working. After it was
deployed, I checked the form properties and neither the 'Send form
definition with item' or 'Use form only for responses' are checked;
which seems to have caused similar problems with other developers.
I've cleared my Outlook cache and removed the form from my Personal
Library to eliminate any potential conflicts but I'm still stumped.

The only thing that I thought of was that once it was deployed with my
designation of v1.2, the Exchange admin made a change and did not
updated the version.

Any thoughts or suggestions would be greatly appreciated.

First thing to do is check whether it's actually one-offing (which means it
won't run code). Both the Size and the MessageClass properties should be
good indicators.
