publishing web forms

troubles with forms

i have created a reply form on my website using publisher template, but the
form does not work. i have the form linked to an e-mail address that sends
and recieves e-mail. i used publisher 2003 for the entire site, all the
links work and everything else but the reply form. any suggestions would be
great.
