QBF somehow disrupts my mail merge

I was easily able to create a mail merge document based on a query. However, once I started using Query by Form, my mail merge no longer works. It instead asks me to select which table I want to use before it opens my Word Document.

If I remove the QBF criteria from the query, the mail merge works but it creates a merge for every single record on my form, which I don't want.

I also noticed that when there's a cQBF criteria on my query, the query is no longer listed as a choice of sources when I start creating a merge form in MS word.

Does anyone know a workaround for this?
