Querying and Reporting From a Form

Not sure what category this should be in -

I have a query that lists projects by year. Example-
Project A - 2004
Project B - 2004
Project C - 2005

I want to be able to select a "Year" or select "All Years" within the query
in a drop down list and generate a report of this information. I would like
to have an "enter" button that will generate the report. I can't seem to
setup the lookup box correctly in the form.
