Quote form

I have designed a quotation form with fields, and have printed several copies
of this form with no entries filled in. I want to be able to insert one of
the blank printed forms into the printer and when i complete a form only the
entries i have entered will print instead of the form layout which i already
have on my printed pages.

Can any one help
