Really annoying - adding a new menu duplicates another one

Why is it when I want to create a new menu on the toolbar (dragging File >
New from the pop-up customise commands) it immediately mimics another
customer-made menu - and when I delete items from the new menu - they also
delete from the old one! Is this a bug?

Try renaming your outcmd.dat file to outcmd.old while Outlook is closed.
Outlook will rebuild the default toolbars the next time you open it (any
changes you have made previously will be lost). If your changes to the
toolbars work correctly after doing this, then you know it was the
outcmd.dat file causing the problem. Make sure to make a backup of the new
outcmd.dat file after you customize it again, so you can restore it if you
ever run into the same problem again.

Note: If you're using Windows 2000 or XP, you'll need to search in hidden
and system folders to find the outcmd.dat file.

All toolbar and menu customizations are stored in the outcmd.dat file
(though the file doesn't hold information from toolbars and menus installed
by 3rd party addins). Yeah, I know, it's a pain when you've made a lot of
changes because there's no way to pull those changes back in from a backup
copy of the file if you allow the default toolbars and menus to be
recreated. You could put the old outcmd.dat file back, but if it was
corrupted you'll just get the same problems again. At least now you know
how to back up a good working file before it gets corrupted...sorry I don't
have a better solution for you.
