Any suggestions as to how these items could have been removed all of a
sudden and how to "bring them back"?

My Tasks / Site Settings / Edit Page / Edit / Modified Shared Webpart /
Project Web Access

Make sure the Menu Bar selection is checked click OK and exit edit mode.......

What I found that is when I hide Timesheets in the Quick Launch (per
Look and Feel section in the Server Settings), the app removes the
tool bar from my tasks and you can no longer see Previous | Next,
etc. When I "unhid" Timesheets, the toolbar reappeared
