Records retain partial information when creating new record! HELP!

I am working with Access 2000 XP Windows. I have a survey that has 1-4
rating and Customer reps names so I created drop boxes with data in the table
portion. To create the list boxes I "change to" List box and then went into
properties box and clicked on Field List and added table included annwers.
When I complete a record (form) and advance to a new one the names of
customer reps carry over every thing else is blank. If I change the names of
cust reps in record 2 it changes it in record one so they are the same. I
checked the properties and they look the same for all drop boxes so why would
it change just the cust reps data? Please help, ITT at work hasn't a clue
and will only reformat Access on my computer..duh!
