Reply action to open Custom form in Outlook 2003

I have a form (A) in which I would like another form (B) to open if when the
user clicks “Reply†after receiving form A. On the action tab for the form
A, I set the following for the Reply action: Both forms are published to my
Personal Forms Library.

Create forms of type - name of my form B with message class “IPM.noteâ€
Address form like – Reply
When responding - Include original text
Prefix - PC

I choose the original form and email it to myself. Then I open the email,
and click Reply. I do see the name of form B in the title bar of the window,
but I don’t see the text on the body of form B.

I have modified the version and re-published. Send Form Definition with Item
is not checked. I deleted all the files in my \localsettings\application
data\microsoft\forms for good luck.

Can anyone help?
