Risks and Issues Lists Attachments

I am trying to attach files to a some items in my Risks and Issues lists. I
am able to edit the item and attach the desired file with no problems. When
I click on Save and Close, everything seems to work normally (no errors.)
However when I view the item, there is no attachment.

Hi Michelle,

after you attached and saved the issue, go the the bottom of the list, there
should be a link with the document name ( not to obious). When you click on
the link, the document should open up
